If your complaint is specific to treatment from another primal therapist, you would substitute their details into the form.
The complaint procedure explicitly states that complainants can not be harassed, and if you are harassed to report it to the board. Complaints can be filed anonymously. However, if you do file anonymously, make sure the complaint is sufficiently documented (for example if all the documentation is on primal therapy websites, or in Janov’s books, make sure you cite those).
To file a complaint on paper, it is less efficient than the online complaint link above. However, you may have good reason to use paper, and in that case use this form:

Note that although the forms seem to be set up for therapist-patient disputes, you can also use them for other types of complaints. For example you could use them to complain that a primal therapist is making false and outrageous claims in their books or websites, that the therapy has iatrogenic properties, of APA ethics violations, or that their therapy does not fit with scientific knowledge.
